New Patients

We are delighted to accept registration requests from individuals regardless of their address although our catchment area encompasses an approximate 5-mile radius around Nettlebed. This coverage encompasses a majority of the region displayed on the map provided below. However, patients outside our catchment area can only receive telephone or surgery appointments; home visits are not available. During peak demand, we may not be able to accept registration from patients outside the Nettlebed area.

To register with our practice, you will need to complete registration forms, which can be obtained from our reception desk.

If you prefer to be registered with a specific doctor, please let us know, otherwise we will allocate a doctor for you. You have a right to be informed of which doctor this is. If you would like to know who is your named doctor please contact reception and they will be able to tell you.

Anyone in England can register with a GP surgery. You do not need proof of address or immigration status, ID or an NHS Number.
